Position Description:Physical Therapist

Home Health Setting


This position offers a $500 Sign on Bonus or Paid CEUs


Responsibilities:

Demonstrates through knowledge of the principles and practice and provide therapy services in accordance with the physician's plan of care.

Conducts initial evaluation and consults with physician to develop treatment plan and reevaluates patient every two months and upon discharge

and documents evaluation

Performs physical therapy procedures, measurements, evaluations and diagnostic tests on patients

Plans and administers physical therapy treatments, i.e., manual therapeutic exercises, non-manual exercises, application of physical agents.

Translates all exercises to patient/family/staff into functional activities or activities of daily living.

Provide training and supervision to home health aides as necessary and documents instructions on a timely basis. Assesses and documents aides

competency in performing assigned exercises, etc.

Requirements for this job:

Bachelor Degree and graduation from a program approved by the Council of Medical Education of the American

Current or pending state license in practicing state

Ability to make home visits and provide physical therapy evaluations and treatments in the home environment
