Responsibilities:


- Responsible for evaluating/assessing needs of referred patients and formulating treatment plans.


- Provide therapy services, work cooperatively with physicians and case managers.


- Provide manual physical therapy to patients.


Must have skills:


- Must have Physical Therapy License or be a Certified Physical Therapist Assistant or a graduate of an accredited therapy program


- Experience working for an outpatient care facility (preferred)


- Must be personable, energetic and great at building relationships with customers


- Current or pending state license in practicing state
